چکیده

Neoplastic diseases account for approximately 13% of the annual incidence of pleural effusions, and 75% of these effusions are secondary to malignancies of the lungs and breast or lymphoma. The most common pathogenic mechanisms that produce malignant pleural effusions (MPE) are (1) pleural metastasis resulting in increased membrane permeability beyond the capacity of lymphatic drainage, (2) metastatic disease of the lymphatic system resulting in decreased clearance of pleural fluid, (3) bronchial obstruction resulting in reduced pleural pressure, and (4) pericardial metastasis resulting in pleural fluid accumulation.2

Talc mediates angiostasis in malignant pleural effusions via endostatin induction.

Talc remains the most effective sclerosing agent for pleurodesis. However, its mechanism of action in resolving pleural malignant disease remains unclear. The present study evaluated the angiogenic balance in the pleural space in patients with malignant pleural effusions (MPE) following talc insufflation. Patient pleural fluid samples were collected both before and after talc insufflation. The ...

Talc pleurodesis for the management of malignant pleural effusions in Japan.

OBJECTIVE Malignant pleural effusions are commonly treated with tube drainage followed by chemical pleurodesis to maintain the patient's quality of life. While talc is now accepted to be a worldwide gold-standard sclerosing agent for treating malignant pleural effusion, it is not yet approved in Japan. Instead, many patients are administered OK-432 for pleurodesis, which carries the risk of com...

Intrapleural talc for the prevention of recurrence in benign or undiagnosed pleural effusions.

Chemical pleurodesis has become the preferred treatment for definitive management of malignant pleural effusions. The treatment of patients with recurrent benign or undiagnosed pleural effusions, however, remains a difficult clinical problem. Tetracycline has been widely used as a sclerosing agent, but parenteral tetracycline is no longer available. Therefore, alternative sclerosing agents are ...
